Objective: The aim of this study is to investigate whether preexisting psychiatric conditions are a predisposing factor for the development of a delirium during IC admission. Background: Delirium is an important factor of the Post-Intensive Care Syndrome. There is evidence that depression and psycho-pharmacy have an impact on the incidence of delirium, where an increased mortality and morbidity has been observed in psychiatric as well as in delirious patients. Methods: Single-center descriptive retrospective cohort study in the Intensive Care Unit in Medisch Centrum Leeuwarden. Included patients were at least 18 years old and had a length of stay ICU of at least 48 hours in the period of 2012-2013. Patient data were collected from Metavision and Mirador. Results: Delirium occurred in 267 study participants (59%). 92 of the 456 (20%) participants had preexisting psychiatric conditions. In a multivariate regression model 6 risk factors for delirium were identified. These risk factors included age (Odds ratio [OR] 1,029; 95% confidence interval [IC], 1.01-1.05), female sex (OR, 0.57; 95% CI 0.38-0.88), SOFA score in the first 24 hour (OR, 1.14; CI, 1.04-1.25), APACHE III (OR, 1.02; CI, 1.01-1.03), number of medications used at home (OR, 0.95; CI, 0.90-0.99) and preexistent psychiatric condition (OR, 1.98; CI, 1.16-3.38). The R2 of this model was 17,4%. Conclusions: A majority of long-stay ICU patients suffer from delirium during ICU admission. A preexisting psychiatric diagnosis is associated with an almost 2-fold increase in the development of such delirium.
