Abstract
Introduction: Patients with kidney failure have an increased risk of developing foot ulcers. The risk of an ulcer is even higher for patients on dialysis therapy. Diabetes is also a major risk factor for foot ulcers. Diabetic foot ulcers are associated with higher mortality. Not much is known about the effect of foot ulcers on mortality in a population of patients with kidney failure and with or without diabetes. Methods: In this retrospective study, the effect of foot ulcers on five-year mortality was investigated in a cohort of predialysis patients (N=683) and patients on dialysis therapy (N=278). Patients with and without diabetes were included. All patients had a follow-up period of at least a year. Kaplan-Meier analyses were performed to compare mortality rates between ulcer and nonulcer groups after five years. Multivariable regressions were developed to adjust for confounders. Results: In the dialysis group, five-year mortality was higher for patients with an ulcer, compared to patients without an ulcer (resp. 78% and 42%, P=0.001). Even after adjusting for variables like diabetes, the difference was significant. No significant difference was found between patients with or without an ulcer in the predialysis group (resp. 26% and 24%, P=0.44). Conclusion: An ulcer is an independent risk factor for mortality after five years for patients on dialysis therapy, but not for predialysis patients.
