Abstract
Introduction: Oppositional defiant disorder (ODD) and conduct disorder (CD) are psychiatric disorders diagnosed in children and adolescents, characterized by a repetitive and persistent pattern of negative, defiant and hostile behavior. The DSM-IV (Diagnostic and Statistical Manual of Mental Disorders) distinguishes two types of CD when looking at the age of onset: childhood-onset and adolescent-onset CD. Of the two, childhood-onset CD is associated with more severe conduct problems. Recently, another specifier, presence of Callous-Unemotional traits (CU-traits), has been added to the diagnosis of CD in the DSM-V. The presence of CU-traits is often related to a more severe form of CD. Both age of onset and CU-traits try to distinguish subgroups that show a difference in the severity of conduct problems. The severity of CD is expressed by the presence of aggression, which can be divided in two types: proactive and reactive aggression. Aim of this research: To investigate whether proactive aggression and CU-traits are predictors of childhood onset CD and, in addition, to examine the influence of reactive aggression on this prediction was examined. Methods: Data was obtained from a sample of adolescents admitted to a closed health care institution. Diagnosis of CD was assessed using the DISC (Diagnostic Interview Schedule for Children). The presence of CU-traits was measured using the YPI (Young Psychopathic Traits Inventory) questionnaire while pro- and reactive aggression was measured with the RPQ (Reactive-Proactive Aggression Questionnaire). All measurements were completed within 6 weeks after admission date. Results: In total, 475 persons have been included in this study. Children diagnosed with childhood onset CD showed significant more CU-traits compared to those diagnosed with adolescent onset CD. No significant results were found between both forms of aggression and the type of CD. Neither CU-traits, proactive nor reactive aggression did significantly predict the age of onset. Conclusion: CU-traits are more strongly related to childhood onset CD than adolescent onset CD. However, similar results have not been observed for the relationship between proactive or reactive aggression and the age of onset. CU-traits, proactive aggression and the combination of both appear not to be of predictive value for the diagnosis childhood onset CD.
