Abstract

Background: In vulnerable elderly, patients with cognitive symptoms have a high risk of medication related problems. The use of potentially inappropriate medication (PIM) can lead to declined function, both cognitive and physical. Little research has been done in this area. Objective: To describe the use of PIM in community-dwelling elderly with dementia in the Netherlands. Methods: In this exploratory, descriptive cross-sectional study the medical records of 200 community-dwelling elderly were analysed. To identify PIM several lists were used: the STOPP-criteria, the Anticholinergic Drug Scale (ADS), the Sedative Load (SL) and a list based on literature with possible anticholinergic medication (AC-lijst). Results: The prevalence of PIM when all lists were used summed up to 55% in all patients, and 63,2% in patients who used at least one drug. The prevalence of PIM based on the STOPP-criteria was 27,5% in all patients and 31,6% in drug users. De prevalence of PIM based on the ADS was 36,0% (drug users 41,4%), based on the AC-lijst 48,5% (drug users 55,7%), and based on the SL 32,5% (drug users 37,4%). A total of 28 patients scored at least one point on each of these lists. Conclusion: A high proportion of community-dwelling elderly with dementia used some type of PIM. Medication use should be an important point of interest in the care for elderly patients with dementia.
