Abstract

Background: Vitiligo is a common skin disorder characterized by depigmented lesions due to loss of melanocytes. Until today there is no sufficient treatment for this disfiguring disease and further research is performed. Nowadays patient reported outcomes are becoming more important in the evaluation of treatment success. As a result, self-assessment tools have been developed, like the SA-PASI for psoriasis and PO-SCORAD for atopic dermatitis. Selfassessment will increase patient’s insight in their disease, improve communication between the patient and doctor and hopefully increase patients’ treatment commitment. However, in the measurement of vitiligo a self-assessment tool is still missing. Based on the VASI we developed the first self-assessment tool to assess the degree of depigmentation in vitiligo: the SAVASI. Objective: Primary, to assess the validity, reliability and acceptability of the SAVASI. Secondary, to evaluate the correlation between the ability of the patient to assess depigmentation with the SAVASI with the quality of life. Methods: Until now we included 55 patients with vitiligo who had an appointment scheduled in the Netherlands Institute for Pigment Disorders (SNIP). The VASI was scored by the physician. Before or after their appointment patients watched an instruction film and then filled out the first SAVASI questionnaire directly. They filled out the second SAVASI after two weeks. The validity was assessed by calculating the ICC between the VASI and SAVASI, the intra-observer reliability by calculation the ICC between the two SAVASI scores. To assess the acceptability, the patients were asked to fill out the time it took to complete the SAVASI and how difficult they found the questions. The VIDA-score was used to assess the disease activity, the Skindex-29 to determine the quality of life. Results: The results are preliminary and based on 55 VASI and SAVASI scores, and 19 second SAVASI scores. There is a high correlation between the VASI and SAVASI (n=55, ICC: 0.97), and a high intra-observer reliability between the two SAVASI scores (n=19, ICC: 0.73). 83,4% of the patients completed within 10 minutes and only 7,3% considered the SAVASI hard. There is no correlation between over-or underestimation of the SAVASI score and Skindex-29 score (ICC = 0.05) , nor is there between the VASI score and Skindex-29 (ICC = 0.04) Conclusions: The SAVASI is a valid, reliable and acceptable self-assessment tool to measure the degree of depigmentation in vitiligo. The SAVASI scores patients measure are not influenced by their quality of life.
