Aims: Weight gain is a well-known effect of insulin intensification in persons with type 1 diabetes (PwT1D). Hybrid closed loop (HCL) systems provide tight glucose regulation, but little is known about the effects on body weight. This retrospective study aimed to evaluate changes in body weight after initiation of HCL-therapy in PwT1D. Methods: A total of 520 patients initiating a HCL-system were included. Of these, 180 patients were matched to 180 controls. The primary outcome was the difference in weight trajectories between patients initiating a HCL-system and matched controls without a HCL-system. Secondary outcomes included weight trajectories up to one year and determinants of weight change, including differences between HCL indication subgroups (hypo-unawareness and inadequate glycemic control). Analyses were done using linear mixed models, supplemented by Pearson correlation analyses. Results: Initiation of a HCL-system was not associated with a significantly different weight trajectory compared with matched controls (p = 0.15). A significant increase in weight was found of 0.89 kg at six months (p = 0.033) and 1.80 kg at one year (p < 0.001) relative to baseline. No significant difference was found between subgroups. Older age was associated with less weight gain (p = 0.002). HbA1c change was not a significant predictor of weight change. Conclusion: Body weight trajectories of patients initiating a HCL-system and matched controls did not differ significantly, suggesting that the observed weight gain cannot be solely attributed to the HCL-system. HCL indication subgroups did not differ significantly in weight trajectory
