Abstract

Background and Purpose: A mastectomy is the foundation of the curative treatment for breast cancer but is associated with a worsening of quality of life (QoL). Breast reconstruction may improve the QoL. An innovative method for total breast reconstruction is Autologous Fat Transfer (AFT). This study aims to examine the effect of total breast reconstruction using AFT on QoL after 12 months. Materials and Methods: This is an observational, multicenter, prospective cohort study of 232 patients who underwent breast reconstruction using AFT. The primary outcome measure is the change in QoL, using 10 domains of the BREAST-Q and BODY-Q, after 12 months. The results were analyzed using a paired t-test. Results: The mean difference in score per QoL domain is for psychosocial well-being -6.37 (p < 0.001), sexual well-being -8.70 (p < 0.001), physical well-being 1,73 (p = 0,591), satisfaction with the chest area -17.11 (p < 0.001), the abdomen -12.61 (p < 0.001), the back -10.11 (p < 0.001), the body -9.18 (p < 0.001), the hips and outer thighs -11.21 (p < 0.001), the inner thighs -8.52 (p < 0.001) and with the scars -6.22 (p = 0.003) Discussion and Conclusion: A breast reconstruction with AFT leads to a significant increase in QoL in three of the four domains of the BREAST-Q and all of the six domains of the BODYQ. This shows that AFT is effective as breast reconstruction. It offers an alternative for women who want breast reconstruction after mastectomy and prefer an autologous and less invasive treatment option.
