Abstract

Background: In the past the value of the sentinel node (SN) procedure of head and neck melanoma’s has been highly doubted by many experts and was therefore not offered by every hospital. However, in November 2018 the Dutch government approved adjuvant therapy for patients with proven nodal metastases causing a significance change in importance of the procedure. The goal of this study was to investigate the success rate and morbidity of this procedure, moreover to identify possible tumor- or patient characteristics with significance influence on these outcomes. Methods: A Dutch national pathology system was used to include all patients between January 1 2013 and July 24 2019 treated in three Dutch hospitals with a primary head and neck melanoma, classified as IB up to and including IIC. Results: A total of 277 patients were included, 51.2% chose not to undergo a lymphoscintigraphy. This group of patients had a significant higher age (69.53 versus 59.14, p<0.01). At least one SN was identified by lymphoscintigraphy in 93.1% and in five cases (2.9%) there was no SN identified. The success rate of the procedure was 72.3%, in 18.5% the operation was cancelled due to the results of the lymphoscintigraphy. No patient- of tumor characteristics had a significance effect on the success rate (p>0.02). The morbidity of the procedure was 5.7% (n=3). All the complications were transient and classified as grade one according to the Clavien-Dindo classification. Conclusion: The sentinel node procedure of melanoma in the head and neck region has a success rate of 72.3%, with a low chance of morbidity and can be offered to every patient.
