Abstract

Background: Polypharmacy is frequent among elderly patients with cancer and it increases the risk of adverse drug events, drug-drug interactions and hospitalizations. This can decrease the patients’ satisfaction with their medication and their quality of life. Polypharmacy assessments aim to reduce these potential drug-related problems (DRPs). A patient-oriented measure for the effect of a polypharmacy assessment is the patients’ satisfaction with medication, for which the ‘Treatment Satisfaction Questionnaire for Medication’ (TSQM) can be used. This study aimed to evaluate the effect of the OncoSTRIP polypharmacy assessment on patient satisfaction with their medication, using the TSQM. Methods: Patients aged 65 and older with five or more chronic medications who visited the outpatient oncology clinic of Medisch Spectrum Twente were included. Patients first answered the baseline TSQM, subsequently the pharmacist conducted the OncoSTRIP polypharmacy assessment and one month after the pharmacotherapeutic recommendations were discussed with the patients, the follow-up TSQM was answered. Results: Fourteen patients participated in the study. Even though there was a trend towards somewhat better TSQM scores after the medication assessment, the differences observed were not statistically significant in any of the four domains (effectiveness, side effects, convenience, global satisfaction). With the OncoSTRIP, a total of 29 potential DRPs were found in 12 patients (86%). In total, 28 optimisations were recommended, with an acceptance rate of 64%. After follow-up, 83% of the implemented recommendations were maintained. It was possible to reduce the number of DRPs by 52%. Conclusion: The OncoSTRIP polypharmacy assessment has no significant effect on the satisfaction with medication in elderly patients with cancer. The assessment identifies many pharmacotherapeutic optimisations.
