Abstract

Objectives: This study aimed to identify a potential difference in the quality of the transurethral resection of a bladder tumour (TURB) procedure between urologists and residents in training at the Spaarne Gasthuis Hospital, Hoofddorp/Haarlem. Methods: A retrospective study was conducted using data of 906 patients who underwent a TURB procedure, in the period between 1-01-2019 and 30-06-2021. After exclusion 878 TURB procedures from a total of 661 patients could be included in the study. Data on sex, age, BMI, tumour characteristics, complications and follow-up cystoscopy were collected. Presence of detrusor muscle in the resection specimen was taken as primary outcome parameter for quality of the TURB procedure. Secondary outcomes were suspicion of recurrence/residual tumour after follow-up cystoscopy at three months after TURB, and postoperative complications according to the Clavien-Dindo classification system. Multivariable logistic regression was used for statistical analyses using IBM SPSS statistics 26. Stepwise adjustment for confounders was made (tumour stage, tumour grade, sex, age, operation code). Results: There was no statistically significant difference in the presence of detrusor muscle (p=0.481), complications (p=0.786) and suspicion of recurrence/residual tumour at follow-up cystoscopy after three months (p=0.526) between the TURB procedures conducted by the urologists and the residents. This lack of statistically significant difference between the operators remained after adjusting for confounders, such as tumour stage, operation code, sex and age (presence of detrusor muscle (p=0.806), in complications (p=0.516) and in suspicion of recurrence/residual tumour (p=0.509)). Tumour stage and grade were not confounders in the correlation of recurrence/residual tumour and operator. Conclusion: This study concludes that there is no difference in the operation quality between urologists and residents in training when conducting a TURB procedure. This result may highlight the good quality of the training of urology at Spaarne Gasthuis Hospital.
