Background: Lung cancer is the most common cancer in the world and the leading cause of cancer death. In the past decade, immunotherapy with immune checkpoint inhibitors (ICIs) proved to be a game changer in treatment of advanced non-small-cell lung cancer (NSCLC). Smoking is associated with poorer health-related quality of life (HRQoL), but little is known about the impact of smoking status on HRQoL in NSCLC patients treated with ICIs. In this study, we sought to evaluate the association between smoking status and HRQoL six months after start of ICI treatment in NSCLC patients. Methods: Patients were adults from the OncoLifeS data-biobank diagnosed with NSCLC, received ICIs at the University Medical Center Groningen (UMCG), the Netherlands. The European Organization for Research and Treatment of Cancer Quality of Life Questionnaire C30 (EORTC QLQ-C30) was used to evaluate the quality of life of lung cancer patients. Participants were divided into three smoking status groups: current smokers, former smokers, and never smokers. The association between smoking status and HRQoL was assessed using generalized linear regression analyses. Results: A total of 157 patients were included. 101 (64%) were men. 128 (81%) patients had stage IV disease and 131 (83%) received mono-immunetherapy. Neither global health status nor summary score was significantly associated with smoking status at six months follow-up. Conclusion: These findings suggest that HRQoL is not significantly affected by smoking status during treatment
