Abstract

Background – People with low oral health literacy experience difficulties in understanding, appraising and applying information about their oral health. This can result in lower oral health outcomes. On average, one in four Dutch people has low health literacy, but the prevalence of low oral health literacy in the Netherlands is still unknown. The aim of this research is to provide insight in the level of (oral) health literacy in the patient population of the Center for Dentistry and Oral Hygiene (CTM). The research questions are: (1) What is the level of (oral) health literacy in patients of the CTM? (2) What is the relationship between health literacy and oral health literacy? (3) What factors predict or are predicted by oral health literacy? Methods – A cross-sectional study was conducted among 204 CTM patients of the CTM Groningen, by using a digital Dutch questionnaire. Enrollment took place by e-mail and at the clinic from 12-1-2020 to 5-30-2021. Self-reported oral health literacy was measured with the Oral Health Literacy Adult Questionnaire (OHL-AQ-NL), health literacy with the Set of Brief Screening Questions (SBSQ-D) and various predictive and dependent factors of oral health literacy by self-developed questions. The level of (oral) health literacy was analyzed by means of descriptive statistics and the association between health literacy and oral health literacy with the Pearsons correlation coefficient. Predictive factors of oral health literacy were analyzed with multiple regression analyzes and the effect of oral health literacy on dependent factors with logistic regression tests. Results – Low oral health literacy was found in 9,7% of the population and low health literacy in 2,9%. Oral health literacy and health literacy showed a positive correlation (p = ,001). Predictors of oral health literacy were education level (p = ,001) and health literacy (p = ,012). Oral health literacy was a positive predictor of interdental cleaning (p = ,028) and a negative predictor of edentulous dental status (p = ,013). Discussion and conclusion – The prevalence of low (oral) health literacy was lower compared to previous research. A possible reason is the extra time and explanation patients receive during their appointment at the CTM. Low oral health literacy was predicted by low educational attainment. Low oral health literacy predicts less interdental cleaning and more frequent complete teeth loss. This study partially finds the expected correlation between oral health literacy and oral health outcomes. Future research among patients on the waiting list of the CTM should show whether treatments on the CTM affect the level of (oral) health literacy. Clear and patient-centered communication aimed at patients with low (oral) health literacy remain important to promote good self-care and oral health.
