Abstract

During pregnancy physiological changes occur in the body, causing gum disease to develop more quickly. If gum disease leads to periodontitis, it can potentially cause negative pregnancy outcomes. That is why oral health deserves extra attention during pregnancy and an (extra) visit to the oral healthcare professional is desirable. The aim of this research is to explore how pregnant women can end up with an oral healthcare professional via the midwife. The following research question was formulated: To what extent do the determinants outcome expectation, task perception, descriptive norm, subjective norm, self-efficacy, attitude and current behavior affect the intention of midwives to systematically address dental check-up of pregnant clients? Although the study focuses primarily on midwives, another study was conducted among pregnant women. A second question was formulated for this: What is the willingness of the pregnant woman to follow the midwife’s recommendation to visit an oral healthcare professional and to what extent does having dental insurance affect this? A cross-sectional study was conducted among midwives. They were recruited through a KNOV conference and by writing to midwifery practices by e-mail. For the study, a questionnaire was developed based on the Theory of Planned Behavior and using the MIDI. Seven determinants that may influence the intention of midwives to address dental check-up of pregnant clients were measured. A total of 52 midwives completed the questionnaire. Multiple linear regression analysis was used to analyze the influence of the determinants on intention. The results showed that the determinants ‘current behavior’ (p=,02), ‘task perception’ (p=,02), and ‘descriptive norm’ (p=,02) significantly affected the intention. For the study among pregnant women, existing data was analyzed from 122 pregnant women. The three variables "dental insurance", "address referral to oral healthcare professional" and "amount of money for dental visit" were analyzed using a chi-square test. There appeared to be no significant association between having dental insurance and the willingness of the pregnant woman to follow the midwife’s recommendation to visit an oral healthcare professional. By responding to the significantly correlating determinants, the intention of midwives to address dental check-up in pregnant women can be increased. Collaboration between midwives and dental professionals will facilitate the referral of the pregnant woman. This also implies a role for the dentist and dental hygienist.
