Abstract
Study objective: To evaluate the effectiveness of thermal balloon endometrial ablation with ThermaChoice III in general practice compared to strict criteria. Design: Retrospective cohort analysis (Canadian Task Force classification II-3). Setting: Martini Hospital Groningen, patient files from January 1, 2011, until January 1, 2014. Patients: 263 premenopausal women with abnormal bleeding patterns who had a treatment with the ThermaChoice III or had a uterus extirpation (UE). Interventions: Data collected by review of patient files included age, BMI, previous treatments, (types of) myomas, bleeding patterns, postoperative complications and treatment, operative parameters and length of follow-up. Effectiveness was measured by postoperative bleeding patterns, complications, additional therapy and duration of hospital admission. Measurements and main results: 136 women received treatment with the TCIII and 127 directly a UE, with an average (maximum) follow-up of 30 months. Strict inclusion criteria with the TCIII do not give a significant improvement of effectiveness. Following TCIII, 86% of the women had a successful outcome (eumenorrhea or less), 11,1% persisting menorrhagia. Following UE more often complications (p=0,01) and a longer duration of hospital admission (p<0,01) were seen. Following TCIII more often additional therapy was started (p<0,01); 11,8% received a UE. Conclusions: Strict inclusion criteria, hormonal pretreatment, different types of bleeding patterns and leiomyomas seem not to be associated with different effectiveness of the TCIII. Following the TCIII more often additional therapy is needed. Following UE more complications and a longer duration of hospital admission were seen. More research is necessary to evaluate the effectiveness of the TCIII in general practice, with a larger group of patients.
