Introduction: The use of laboratory tests in primary care is increasing. Laboratory tests can support general practitioners (GPs) in finding diagnoses. Value and frequency of requesting tests in addition to various symptoms is unclear in primary care. An overview of additional laboratory tests gives insight in current use, and provides leads for further research. The goal of this study is to describe frequency and type of laboratory tests requested by GPs in children. Methods: In this retrospective observational study, data from Academisch Huisarts Ontwikkel Netwerk (AHON) database has been analyzed. For all cases known between January 1st 2016 and January 1st 2020, type of laboratory test, sex, age, International Classification of Primary Care (ICPC) code and general practice were included in the dataset. Using descriptive statistics, an analysis has been made of the laboratory tests that have been requested in children between 0 and 18 years old and which ICPC-codes are linked to these types of diagnostics. A top 10 has been made of the most frequent requested types of laboratory tests and ICPC-codes. Results: A total of 359.567 laboratory tests have been requested in 20.161 (25,2%) children during 5 years. More laboratory tests were requested in case of girls and children ≥12 years old, compared to boys and children <12 years old. The top 10 existed of hemoglobin (4.8%), leukocytes (4.5%), C-reactive protein (CRP) (3.8%), thyroid stimulating hormone (3.2%), creatinine (2.9%), thrombocytes (2.8%), alanine aminotransferase (2.3%), sedimentation (1.9%), gamma-GT (1.9%) and urine stick test (1.7%). Most requests regarded the ICPC-code fatigue/weakness (A04) (25.3%) Conclusion: Hemoglobin is the most frequent laboratory test requested in children. Next to hemoglobin, determinations belonging to general laboratory diagnostics and infection parameters like CRP and leukocytes are frequently requested. Fatigue/weakness (A04) is the most commonly assigned ICPC-code in laboratory tests, although the effectiveness is unknown.
