Abstract

Introduction: The problem is that there has been a few studies who has researched implant supported single-’unit’ crowns with a mesial or distal cantilever. Because of the small amount of studies there is little known about the survival of this construction and if load on this construction would lead to complications. The question the researcher would like to answer is: what is the clinical performance and survival of implant supported single-’unit’ crowns with a mesial or distal cantilever in the posterior zone of maxilla or mandible after at least one year of function in patients who are treated in an implantology practice in Apeldoorn and how satisfied are those patients? The aim of this study is to provide an evidenced base for this construction in order to possibly implement the results to the clinical setting. Material and methods: In this retrospective study patients who got an implant supported single- ’unit’ crown with a cantilever between January 2004 and February 2018 have been selected. The researcher has done dossier research for each patient, also there has been a control appointment. A few measurements have been taken during that appointment and if necessary an X-ray have been taken. Besides that, two questionnaires have been answered by patients. The most important parameters that have been measured were: marginal bone loss, technical and biological complications of implants and crowns and patient satisfaction. Results: There has been 23 patients with 28 implants selected. Mean age has been 64.3 ± 13.4 years and implants have been 6.5 ± 4.8 years (range 1.3-14.2 years) in function at the moment of research. Marginal bone loss mesial of implants has been 0.41 mm (SD 1.18 mm). Marginal bone loss distal of implants has been 0.63 mm (SD 0.98 mm). Also marginal bone level mesial and distal is significant lower at the moment of research compared to the moment of implanting. Patients have had a mean score of 93.6 ± 11.4 points (maximum of 100 points) at all VAS questions. Furthermore, mean score at total OHIP-NL 49 has been 10.8 ± 8.5 points of 196 points in total. Discussion: Present results are compared to completed studies. There is a significant difference between marginal bone loss in the present study and the mean marginal bone loss after one year of function reported by another study. Possible influential factors on marginal bone loss like position of implant and type of fixation of the crown are not different compared to completed studies. Limitations of this study are: retrospective research, representativity and no control group. Strengths of this study are: relatively big patients group, relatively long follow-up and patients have been treated by one practioner. Conclusion: Implant supported single-’unit’ crowns with a mesial or distal cantilever can be a good treatment in cases where two (pre)molars are missing in upper or lower jaw. This retrospective study has proven that there are a few biological or technical complications during a mean of 6.5 years of function and patients are very satisfied about researched construction. When interpreting the results it should be taken into account that it is a retrospective study. A randomised prospective study should be done to reinforce the evidence.
