Background: The traditional periodontal treatment, scaling and rootplaning (SRP), can be followed if necessary by periodontal surgery. The adjunctive effect of periodontal surgery for patients after initial therapy using non-surgical periodontal treatment and/or systemic antibiotics on the clinical parameters of periodontal disease: clinical attachment level (CAL) and probing pocket depth (PPD) is till so far not evaluated in a systematic manner. Therefore, the aim of this research paper is to evaluate the adjunctive effect of periodontal surgery for patients after initial therapy using non-surgical periodontal treatment and antibiotics by a systematic review. Title of research project: The adjunctive effect of periodontal surgery for patients after initial therapy using non-surgical periodontal treatment and/or systemic antibiotics. Material and Methods: The PubMed-MEDLINE, Cochrane Central Register of Controlled Trials (CENTRAL) was searched to identify appropriate studies. PPD, CAL and Bleeding on Pocket Probing (BOP) were selected as outcome variables. Data were extracted from the eligible studies, and a descriptive analysis was performed for the following evaluations: 1)SRP+SURG+amx+met compared to SRP+SURG, 2)SRP+SURG+amx+met compared to SRP+amx+met, 3) SRP+SURG compared to SRP+amx+met. Results: A total of 53 unique articles were found, 4 studies met the eligibility criteria. Three comparisons evaluating the adjunctive effect of systemic antibiotics on SRP and periodontal surgery. Two comparisons evaluating the adjunctive effect of periodontal surgery antibiotics on SRP and systemic antibiotics. One comparison evaluates the adjunctive use of periodontal surgery compared to systemic antibiotics. The descriptive analysis showed that there is for both periodontal surgery and systemic antibiotics a small adjunctive effect. Conclusion: Despite the caveats concerning the heterogeneity of experimental designs there is weak evidence that SRP+SURG+amz+met shows significantly superior clinical outcomes in terms of PDD and CAL compared to SRP+SURG and SRP+amx+met.
