Aim: To investigate whether 125I-seeds can be used for co-registration of dynamic contrast enhanced magnetic resonance imaging (DCE-MRI) and ultrasound for the fusion of these modalities in breast cancer. Secondary, tumor deformation from prone to supine position is evaluated. Subjects and methods: In the operating room, an electromagnetically tracked 3D ultrasound was acquired in patients that were scheduled for breast conserving surgery using 125I-seeds as an interoperative tumor localization method, that had received preoperative DCE-MRI. Segmentations of the tumor and 125I-seed on both the DCE-MRI and ultrasound were made and registered using 3D Slicer software. After segmentation, the maximum diameter in anteriorposterior (AP), left-right (LR) and inferior-superior (IS) direction were measured in both DCEMRI and ultrasound and compared. Wilcoxon Signed-Rank test and Mann-Whitney U test were used for statistical analysis. Results: A total of 11 tumors, of which 11 DCE-MRI’s and 44 ultrasound acquisitions were included. Segmentations were successful in all DCE-MR images. If the 125I-seed was surrounded by tumor tissue on ultrasound, segmentations were successful. All segmentations in which a tumor had been visible on the ultrasound were successfully registered with the DCEMRI using 125I-seeds. A significant absolute decrease in AP diameter of 33.3% (P=.04), a nonsignificant absolute decrease in IS diameter of 29.2% (P=.08) and a non-significant absolute decrease in LR diameter of 0.9% (P=.89) were shown. Conclusion: 125I-seeds surrounded by tumor tissue can be used as a landmark for co-registration of prone DCE-MRI and supine ultrasound. Tumor deformation from prone to supine position occurs.
