Abstract

Objectives: The objective of this study was to investigate whether there is a relation between the preoperative DETI/CEB score and the radiographic technical quality of root canal treatment (RCT, syn. Endodontic treatment) performed on a referral base by a dentist who works in a differentiated Endodontics practice. Methods: The study was a retrospective observational study. To assess the technical quality of the RCT, pre-operative and postoperative periapical x-rays were used, the statistical unit is the element. The study population includes elements with RCTs performed from 1 January 2012 till 31 March 2014, a total of 227 teeth were evaluated. All RCT are carried out by one dentist, who works on a referral base in a differentiated Endodontic practice. The DETI/CEB score and the number of endodontic treated root canals are already scored by the dentist. Three postoperative variables were used to evaluate the technical quality of the RCT (1) the density of the root canal filling, (2) the length of the root canal filling and of (3) the root canal morphology has been preserved, these three variables are scored by the investigator. In addition, the overall scores were determined, these were only acceptable if all three postoperative variables were rated as acceptable. The results are processed in SPSS, the Chi-square test was used to assess any significant correlations between the results (significance level 0,05). Results: From a total of 227 endodontically treated teeth, 185 (81,5%) had an acceptable technical quality. The length of the root canal filling was found to be acceptable in 89,4% of the endodontically treated teeth, the density was acceptable in 95,5% and the root canal morphology has been preserved in 92,5%. RCTs in elements with one root canal (76,4%) and with four root canals (75,8%) scored lower than RCTs in elements with three root canals (91,6%). The highest technical quality was found in elements with two endodontically treated root canals (95,5%). This relation between the overall-score and the number of treated root canals was found to be significant (α = 1.5%). The remarkably low score of elements with one root canal (76,4%) is due to the high level of difficulty and the high risk of complications during treatment, 47,1% involved a root canal treatment in an element with a DETI/CEB B2 score and 41,2% with a B3 score. The overall technical quality decreases as the DETI/CEB score becomes higher, 90,0% of the elements with DETI/CEB score B1 were found to be acceptable, B2 in 86,6% and B3 in 78,7%. However, only 4,4% of the study population had a DETI/CEB score B1, 29,5% had a B2 score and 66,1% were scored B3. Conclusion: Within the limitations of this study, the results indicate that there is a relation between the DETI/CEB score and the radiographic technical quality of the RCT performed by a dentist who works in a differentiated Endodontics practice. The higher the DETI/CEB score, the greater the probability of an unacceptable technical quality. However, further research is needed to validate these statements. It is recommended to investigate more treatments (with different DETI/CEB scores) and treatments performed by different dentists and in different practices. It is also recommended that the research is conducted by multiple assessors.
