Abstract

Reconstruction of cranio-facial defects consists of transplantation of a free vascularized fibula graft. Nowadays, this surgery is fully 3D planned and cutting/drilling guides are 3D printed. The technical aspects and outcomes of the reconstruction are extensively studied. Less is known about the functional outcomes after surgery. Currently, patients treated with a free vascularized fibula graft in the University Medical Center Groningen are evaluated by questioning how they are doing and if it feels acceptable. A standardized protocol to evaluate functional outcomes is lacking. The aim of this research paper was to create a protocol to evaluate functional outcomes of patients after reconstruction. A search in the database MEDLINE via PubMed was conducted to determine which tests are available to investigate functional outcomes after surgery. The obtained articles were categorized by used method. The following outcome measures were evaluated: mouth opening, masticatory performance, dietary intake, oral continence, swallowing, speech, aesthetics and quality of life. The methods are evaluated on their objectivity, validity and clinical applicability and based on these characteristics a selection was made. The final protocol consists of a combination of questionnaires and easily applicable functional tests. Mouth opening is measured, masticatory performance is evaluated with the mixing ability test and swallowing is studied with VFSS. Other instruments used are the Swallowing Quality of Life, Speech Handicap Index, Drooling Rate Scale and EORTC QLQ C-30 and H&N-35. The VFSS and Drooling Rate Scale will only be used when indicated after completion of the SWAL-QOL. Facial aesthetics is assessed by the patient using a Visual Analog Scale.
