Abstract
Transcatheter Aortic Valve Implantation (TAVI) is a relatively new procedure to implant a bioprosthetic aortic valve into a native valve suffering from severe aortic stenosis (AS). Big trials showed a lower mortality in high risk patients as well as an improvement in quality of life (QOL). So far, no results on QOL have been reported by any of the Dutch heart centers. Our aim was to assess QOL improvements in Dutch high risk patients with severe AS 1 year post-TAVI. We performed a retrospective cohort study with 182 severe AS patients who underwent TAVI in 2009-2015. We used 4 different bioprostheses through 4 different access sites. To assess QOL we used the Short Form 36 version 2 Health Survey (SF-36) at baseline and 1 year post-TAVI. We analysed eventually 78 patients who completed 1 year of follow-up. At 1 year follow-up, significant improvements in physical functioning (28,1 2,7 vs. 46,1 2,9 p 0,001), role physical (25,3 3,1 vs. 43,7 3,9 p 0,001), bodily pain (48,5 2,9 vs. 59,3 3,4 p 0,01), general health (34,3 2,2 vs. 48,6 4,1 p 0,01) and vitality (33, 4 3,6 vs. 46,8 3,9 p 0,001) were observed compared to baseline. No significant improvement could be detected for mental health (58,2 3,1 vs. 60,1 3,4 p 0,05), social functioning (57,4 3,2 vs. 63 3,9 p 0,05) and role emotional (48,3 2,1 vs. 52,1 2,5 p 0,05). The physical and mental scores were transformed into Physical Component Summary (PCS) and Mental Component Summary (MCS). PCS improved significantly in general except for COPD Gold class 3 and 4. No significant improvement could be detected in MCS. In conclusion, this study has demonstrated that TAVI can improve the physical component of QOL with the exception of patients with severe obstructive lung disease.
