Abstract

Background Patient experience is an increasingly important, key quality outcome for health services. The HowRwe is a short, generic questionnaire which aims to measure patient experience. Until now the more laborious EUROPEP questionnaire has been used for this purpose in general practices. Objective We aimed to establish the relation between HowRwe and EUROPEP outcomes. Furthermore, we determined whether the HowRwe questionnaire could predict EUROPEP outcomes. This could provide possibilities in measuring patient experience more easily. Patients and methods A cross-sectional, cohort study was conducted of patients aged 18 or older who were visiting general practices in the Zwolle, Hardenberg and Flevoland region in the Netherlands. HowRwe and EUROPEP scores were collected in two different populations (n=90 / n=1925) and compiled into two databases. Correlation between both scores was studied Spearman’s rank-order correlation coefficients. A HowRwe cut-off value was determined which was most discriminative in selecting patients with a positive EUROPEP outcome. Results Population A: HowRwe total scores and EUROPEP mean scores showed a significant correlation of 0.44. The ROC curve presented an area under the curve of 0.711. HowRwe scores of 12 and ≥ 10 were associated with positive predictive values of 76.2% and 76.7%. Population B: HowRwe total scores and EUROPEP mean scores showed a significant correlation of 0.66. The ROC curve presented an area under the curve of 0.772. HowRwe scores of 12 and ≥ 10 were associated with positive predictive values of 90.1% and 83.4%. Conclusion The HowRwe provides possibilities to be used as a supplementary first step in measuring patient experience. We suggest using the HowRwe questionnaire to single out patients with suboptimal scores (as defined by the user) so that these patients can be asked
