Background: In recent years, an increase in the number of acute referrals of children to the pediatrician has been observed. This study investigates the extent and severity of this increase and analyses the referral patterns by means of a retrospective analysis. In addition, a prospective analysis is performed to investigate the complex and multifactorial causes of the increase. Objective: To objectify and quantify the referral patterns over the years 2012-2016 and to investigate the causes of the increase. By means of a (qualitative) analysis the ideas and views of both patients' parents and the pediatricians are evaluated. Results: Over the years 2012-2016 there has been an increase in the number of acutely referred children of more than 21%. In the years 2014-2016 the increase was as much as 38%. This was mainly due to referrals of young children with respiratory complaints, for which referrals increased by 98% over the period 2014-2016. Especially general practitioners refer more often during out-of-hours. The pediatrician thinks 27% of all acute referrals unnecessary. Parents indicate that previous experiences with a child being ill play a major role in their anxiety and request for care. They frequently search for information on the internet. In some cases, the internet raises more concerns, but more often it confirms their assumption. Conclusion: There has been a very sharp increase in the number of acute referrals, mainly in the young group of children with respiratory complaints. Several factors play a role: increased demand for care from general practitioners in young children, increased anxiety and need for information among parents, changed organization of general practitioner care in the out of hours office, decrease in experience of general practitioners in assessing sick children and changes in hospital organization due to merger of hospitals and changed agreements with parents of recurrent patients on passing the general practitioner and coming directly to the hospital in case of respiratory complaints.
