Introduction: Osgood-Schlatter disease is prevalent in growing children who are actively engaged in sports. The symptoms of these young patients are not easily quantified and this may explain why there are no evidence-based guidelines for managing this injury. A practical questionnaire which evaluates the severity of symptoms in children with Osgood-Schlatter disease would facilitate the study of Osgood-Schlatter disease and can therefore play a role in enhancing treatment and prevention. Aim of the study was to develop such a questionnaire and to study the (test-retest) reliability and (discriminative) validity of this new questionnaire. Material and methods: The Dutch ‘Victorian Institute of Sport Assessment – Patella’ (VISA-P) questionnaire was adapted to become the ‘Osgood-Schlatter Severity Index’ (OSSI) questionnaire. A total of 107 children from local schools with ages ranging from 8 -15 years were included to assess the reliability. These children completed the questionnaire twice with an interval of 2 weeks. To assess the test-retest reliability and the internal consistency, the intraclass correlation coefficient (ICC) and the Cronbach’s alpha were calculated respectively. 15 asymptomatic children, 18 children with Osgood-Schlatter, 14 competitive basketball players, 16 children with other knee injuries and 13 children with diabetes were included to quantify the validity. These children filled out the questionnaire once. The differences between the 5 groups were analysed using the Kruskal-Wallis test and a post-hoc analysis with Bonferroni correction. Result: The OSSI questionnaire showed satisfactory test-retest reliability (ICC = 0.79) over an interval of two weeks. The mean OSSI scores (± SD) were 94.3 (± 6.4) for the asymptomatic children, 57.2 (± 21.7) for the children with Osgood-Schlatter, 93.1 (± 8.4) for the basketball players, 54.1 (± 15.0) for the children with other knee injuries and 92.2 (± 7.5) for the children with diabetes. The mean OSSI score of the children with Osgood-Schlatter differed significantly from that of the asymptomatic children (U = 9; p < 0.001; r = -0.79), the basketball players (U = 242; p < 0.001; r = 0.78) and the children with diabetes (U = 222; p < 0.001; r = 0.76). Conclusion: The OSSI questionnaire demonstrated good test-retest reliability and discriminative validity.
