Abstract

Background: Venous thromboembolisms are important causes of morbidity and mortality following major orthopaedic surgery, which is defined as total hip arthroplasty (THA), total knee arthroplasty (TKA) and hip fracture surgery (HF). Considering recent significant changes in VTE prevention, a study was performed to update the epidemiology of symptomatic VTE following major orthopaedic surgery. Methods: A retrospective study of patients undergoing major orthopaedic surgery between 2014-2018 was performed. The primary outcome was the incidence of diagnosed VTE, defined as deep vein thrombosis (DVT), fatal or non-fatal pulmonary embolism (PE) of both, three months following THA, TKA and HF. Results: Data of 4416 surgeries was available. These consisted of 38% HF, 32% TKA and 30% THA. The mean age of patients undergoing surgery was 71,8 years. The incidence of symptomatic VTE following major orthopaedic surgery was 1.4% (61 cases, 95% CI: 1.01.7), of which 0.4% (95% CI: 0.2-, 5) was DVT and 1.0% (95% CI: 0.7-1.3) was PE. The incidence following THA, TKA or HF was 1.6%, 1.4% and 1.1%, respectively. Three PEs following HF were fatal. Low molecular weight heparin (LMWH) was administered as pharmacological thrombosis prophylaxis to all patients who had developed a VTE. The duration of prophylaxis in these patients was six weeks or until a VTE had developed. Conclusion: The incidence of symptomatic VTE after following orthopaedic procedures is low. However, further improvements in thrombosis prophylaxis are required for more effective protection against VTE for the increasing number of patients undergoing major orthopaedic procedures.
