Abstract

Objective: To investigate the knowledge of healthcare workers when it comes to administering intranasal corticosteroid sprays (INCS). Methods: An observational study was performed in the North of the Netherlands. Healthcare workers included were pharmacist’s assistants, general practitioners, pediatricians and ENT physicians. Healthcare workers demonstrated the administration technique with a spray device filled with water. Stages assessed were preparation, administration and cleaning of the INCS. Assessment of administration technique was based on the officially established ‘corticosteroïde neusspray protocol’. In this protocol, five steps were labeled as essential to obtain optimal distribution of the medication. Healthcare workers also answered a questionnaire that was constructed by the researcher. It contained questions about their experiences prescribing INCS and about how they provide patients with information about this medication. Results: Among the 71 participants included, none performed all stages correctly. The median of correctly performed steps in the protocol was 14 of 29 steps. The essential steps were performed by 35% of the healthcare workers. During preparation, most frequent errors were not activating the spray (41%) and not rinsing the nose (39%). During administration, closing the opposite nostril (54%), spraying crosswise (42%) and holding the head up straight (35%) were the most frequent errors. While cleaning, 48% did not wipe the spraying head after using it and 99% did not advise to fully clean the device on a weekly basis. Conclusion: Healthcare workers do not know how to administer an intranasal corticosteroid spray (INCS) correctly. Patients could therefore receive incorrect and non-uniform instructions from them.
