Abstract

Abstract (English) Introduction: amyloidosis is a group of diseases marked by deposition of amyloid, an aggregate of protein highly resistant to proteolysis. Deposition of these proteins in different organs gives rise to organ dysfunction, leading to oxidative stress. These proteins are expected to be prone to form advanced glycation end products (AGE’s). In turn there is evidence that AGE’s can play a role in the formation of amyloid by destabilizing proteins. AGE’s can be measured in the skin by using their auto fluorescent characteristics. In this thesis we investigated weather amyloidosis patients had higher skin auto fluorescence (SAF) compared to healthy subjects. Methods: 188 measurements were performed in 112 patients. The AGE-reader was used to carry out the measurements. Biomarkers were assessed using routine laboratory techniques. Based on abdominal fat biopsy the degree of amyloid deposition was estimated. Statistical analyses were conducted using paired t-test, Pearson and Spearman tests, ANOVA and Kruskal-Wallis tests and Generalized Estimating Equations. Results: SAF was increased in patients with AA amyloidosis compared to calculated values bases on a validated formula. For the other types of amyloidosis no differences could be found. There was no correlation with degree of amyloid deposition. There was a correlation with kidney function and disease duration, but no correlation with inflammatory status. Discussion: increased SAF in AA amyloidosis patients is most likely a reflection of increased accumulations of AGE’s in an environment of oxidative stress. There are no indications that auto fluorescence of local amyloid caused increased SAF in these patients.
