Abstract

Hyperbilirubinemia is a very common complication in premature born infants. Especially high concentrations of unconjugated bilirubin, which is potentially neurotoxic, can lead to a lot of complications, such as acute or chronic kernicterus. This may ultimately result in severe neurologic dysfunction in later life, such as deafness or cerebral palsy. First choice of treatment for hyperbilirubinemia is phototherapy (PT). PT is supposed to be a safe treatment, but potential side effects are known. Phototherapy may result in vasodilatation, which ultimately may result in reopening of the ductus arteriosus. We hypothesized that PT decreases cerebral oxygenation. Near infrared spectroscopy (NIRS) is a method which measures the cerebral oxygenation. The influence of PT on the cerebral oxygenation will be looked at in a two groups of preterm infants - The infants’ NIRS values will be compared before and after PT. a group of 16 infants born with a gestational age < 37 weeks. - NIRS values of the infants with PT will be compared with NIRS values of a control group without PT in a group of 2x11 infants born with a gestational age <37 weeks. Both studies show that there is a decrease in cerebral oxygenation due to PT, however there is no shown significant decrease in cerebral oxygenation due to PT. PT is associated with a nonsignificant reduction/ decrease of cerebral oxygenation.
