Abstract

Background: Decline in muscle strength in older adults can lead to disability and loss of independence. Older adults with dementia are at risk for these consequences. Strength training programs may improve muscle strength. Adequate participation is a prerequisite to make succeed such programs. Participation can be enlarged by improving the psychological well-being of the participants. Well-being, and consequently motivation and participation, may be influenced by environmental factors. Objective: To investigate the influence of the environmental factors background music, type of instruction, and the exercise room on the psychological well-being, motivation, and participPtion of older adults with dementia in strength training programs. Methods: Seven institutionalized older adults with dementia participated in a 6 weeks strength training program. The exercise environment was manipulated during the strength exercise sessions by presenting background music, a specific exercise room, and action tailored instructions. Psychological well-being, motivation, and participation were assessed by direct observations and video observations of the participants. Results: On average, manipulating the environment did not lead to changes in well-being, motivation or participation. However, individual preferences in environmental setting may be observed. Strong positive interrelations were found between psychological well-being, motivation and quality of execution, and between willingness to participate, attendance and change in strength. Conclusion: In the present sample, the influence of the environmental factors background music, type of exercises, and the exercise room on participation could not be established. Individual preferences in exercise environment may be observed. Because of the low sample size and the limited duration of the program, more research is needed to further establish the existence of individual preferences in environmental setting to find strategies to optimize participation in strength training programs in older adults with dementia.
