Abstract

Introduction. Asthma and allergic rhinitis are both very prevalent diseases, and often seem to occur hand in hand. The Control of Allergic Rhinitis and Asthma Test (CARAT) is developed to evaluate the control of asthma and allergic rhinitis simultaneously. There is recently made a mobile application of the CARAT. The aim of this study is to validate this app. In addition, the current CARAT covers a period of four weeks. This study investigates whether the 1-week CARAT is valid as well. Methods. Patients will be contacted after lung function tests at Certe, an organization for integrated medical diagnostics. Baseline data is gathered here as well. After including, the patient will participate for four weeks. The CARAT app and the 1-week CARAT on paper must be completed each week. During the fourth week, the 4-week CARAT must be completed as well. Averages are tested for significance with Wilcoxon signed rank test. Internal consistency was calculated using Cronbach's alpha. Construct validity was measured using Spearman's correlation coefficient between CARAT and VAS, ACQ, ARIA, GINA. The VAS and ARIA are used as reference questionnaires for the allergic rhinitis part of the CARAT. The ACQ and GINA are used for the asthma part of the CARAT. Intra-Class Correlations (ICC) are measured to determine the test-retest reliability. Results. In total, 127 patients were included. The study was successfully completed by 109 participants. There wasn't found a significant difference between the CARAT app and the paper CARAT (p=0,054-0,858). Cronbach's alpha ranges between 0,778 and 0,877 (app), 0,755 and 0,873 (paper). Correlation coefficients for test-retest reliability range between 0,838 and 0,961 for the paper CARAT and between 0,857 and 0,959 for the CARAT app (p<0,001). Spearman's correlations for the construct validity of the 1-week CARAT range from -0,572 to -0,751. Comparison of the average of the four measurement moments of the 1-week CARAT and the 4-week CARAT gives a p-value of 0,004. Conclusion. This is the first study that investigates the validity of the mobile application of the CARAT. The results show that the 1-week CARAT is a valid and reliable test for the evaluation of the control of asthma and allergic rhinits. However, it doesn't necessarily give the exact same results. In addition, the CARAT app is found to give no significant different results as the paper CARAT. This means that the app can be used just as well as the paper version of the CARAT
