Abstract

Introduction Forearm fractures are the most common fractures in children worldwide. Distal and diafyseal fractures comprise almost half of all fractures at child’s age. The incidence of wrist fractures, tends to increase the past 10 years, especially in boys. The forearm fractures in children attended to in the Onze Lieve Vrouwe Gasthuis in Amsterdam are not treated according to a specific protocol. The purpose of this study was to assess the long-term functional results of children attended to in the OLVG. Other objectives were to give an overview of the proportions of different types of forearm fractures en to evaluate the treatments according to fracture types. Methods 223 children under 16 treated admitted to our trauma centre between January 2004 and September 2008 due to a forearm fracture were included. Of these patients type of injury, background factors, radiographics, treatments and re-dislocations were reviewed. 197 patients received questionnaires concerning the function of the hand/forearm and the quality of life. Results 76 patients (39%) returned the questionnaires after a median follow-up of 5 years. This group of patients was an adequate representation of the total population. In 31% of the population secondary displacement occurred. The initial treatments performed in this group were significantly different (p < 0.001) from the total population. In 15 patients (7%) reintervention was necessary because of secondary dislocation after initial treatment. In 4 patients (1.8%) refracture occurred. In the OLVG between 15 and 25 degrees was accepted for distal fractures. In diafyseal fractures the maximal accepted angulation was 18 degrees for children younger than 9 years. In older children no angulation was accepted. The functional results were equal or better than the reference values. Ages were significantly different (p = 0.02) in children with unacceptable functional results. Conclusion Despite the lack of a protocol and consensus between the surgeons of the OLVG a uniform policy was pursued in the OLVG. The functional results of this executed policy were acceptable.
