Abstract

Background: Impaction of the maxillary permanent canines is a frequently occurring dental anomaly. Substantial research on the treatment of canine impaction over the past few decades has resulted in the adoption of two treatment modalities: The modified window technique and the closed eruption technique. Although both methods are extensively described, it remains unclear which method is the most successful for treatment of canine impaction. The purpose of this study was to evaluate the closed eruption technique. Method: A cohort of 73 patients (mean age 14,6 years; range 11-38) underwent surgery by exposing the impacted canine, fixing a bracket on the exposed tooth and repositioning the gingival flap. Three months after cessation of the orthodontic treatment, the patients were clinical and radiologically evaluated on periodontium, abnormalities in adjacent teeth (periodontium, bone resorption, periapical inflammation) and occlusion. The control population consisted of 70 patients (mean age 14,4 years; range 13-18) with a regular orthodontic treatment without canine impaction. The outcome was statistically tested by Chi-square and the T-test. Correction for confounding has been done by the logistic regression analyses (SPSS 16). The p-value <0.05 was considered statistically significant. Results: The overall success rate of the treatment was 63%. The occurrence of periodontal pockets in adjacent teeth was significantly related to a higher age (p=0,041) and the number of pockets of the treated cuspids was significantly higher in the tested population (p=0,006) after correction for confounding (location and gender) (oddsratio: 5,2 by 95% BI: 1,70- 15,665). Conclusion: Although the closed eruption technique induces a higher risk for the impacted cuspid, it remains a predictable method for exposing the impacted teeth. The overall success rate of this study is in accordance with the literature.
