Abstract
Aim: The aim of this exploratory study is to determine parameters which are probably predictive for posterior urethral valves with regard to boys with urinary incontinence. Material and methods: We retrospectively reviewed data of 72 boys between 5 and 18 years old, who underwent an urethrocystoscopy between 2012 and 2016. Because of an unexpected small control group we decided that further analysis should be focused on the 60 boys with only urethral valves. The data was screened on presenting symptomatology, bladder diary, uroflowmetry and radiographic findings. Also, a sub-analysis was performed in which differences between boys with a rapid diagnosis (<180 days) and boys with a late diagnosis were examined. The resulting data was tested by the Chi-Square test, Fisher’s Exact test and the Mann Whitney Test. Results: The most common symptoms at presentation were urgency (44 patients, 73%) and voiding postponement (38 patients, 65%). Increased urinary frequency was observed in 61% of the boys. 75% had a positive respons to anticholinergics. Ultrasound examination showed no abnormalities in most boys. An extended voiding time occurred in 61% and 55% of the uroflowmetry curves was not bell-shaped. In the sub-analysis the average age in the group with a rapid diagnosis was significantly higher than in the group with a late diagnosis. The symptom interrupted urinary stream and an open bladder neck during ultrasound examination were significantly more frequent in the group with a rapid diagnosis. Conclusion: Boys with incontinence caused by urethral valves often do not show any symptoms or signs. Specific symptoms are rare. This makes it difficult, even for experienced clinicians, to make a distinction between urethral valves or another cause for the symptoms of incontinence. Urge, voiding postponement, reduce of symptoms when using anticholinergics, extended voiding time and a curve other than a bell shape were frequent findings related to boys with urethral valves and may be predictive for the presence of urethral valves. To validate these potential predictive values it is essential to compare the results with the results of a control group consisting of a group of boys with incontinence, but without urethral valves. Also, a larger survey would be desirable.
