Abstract

Introduction: A reduced fetal urine production (FUP) could be an early sign of fetal hypoxia. To calculate the FUP serial fetal bladder volume measurements should be performed. Stigter et al created a 2D ultrasound method to calculate fetal bladder volume, derived from a well validated method introduced by Hedriana and Moore. Currently fetal bladder measurements are frequently performed by 3D ultrasound method which uses VOCAL software (Volume Organ Computer-aided AnaLysis). The 3D method is considered more reliable in comparison to the 2D method. This consideration is used as an argument to reduce the number of volume measurements when calculating FUP using the 3D method. The aim of this study is to investigate the agreement between the 2D and 3D method to calculate fetal bladder volume. Secondary aim is the investigation of the reproducibility and practicability for both methods. Methods: Ultrasound measurements of the fetal bladder were performed in 78 pregnant women. Fetal bladder measurements are calculated by both methods. Ultrasound scans and analyses were performed by multiple observers to investigate reproducibility. The possibility a correct interpretation of the ultrasound scans was assessed to investigate the practicability. Agreement between the methods and reproducibility were evaluated using Bland and Altmann plots. Results: There is no good agreement between the 2D and 3D method to calculate fetal bladder volume. The reproducibility of both methods is comparable. The reproducibility when measurements are performed by 1 person is better using the 2D method. Within the 2D method performing the ultrasound is causing most of the variability. Within the 3D method analysing the images is causing most of the variability. Correct interpretation of ultrasound scans was possible more often when using the 2D method in comparison with the 3D method (87% vs 68%). Within the 2D method possibility of correct interpretation was influenced by maternal BMI. Within the 3D method possibility of correct interpretation was influenced by the duration of amenorrhea. Conclusion: When calculating FUP fetal bladder volume measurements should be performed using the 2D method. It is uncertain which of the methods is more valid. When measurements are performed by 1 person the 2D method has a higher reproducibility in comparison with the 3D method. Also the 2D method has a higher practicability and is independent from the duration of amenorrhea. When calculating FUP the number of volume measurements should be a minimum of 6, as validated by Hedriana and Moore.
