Abstract

Aim: An in vitro study to evaluate the influence of a brushing motion after apical root canal preparation through push-pull movement using the master apical file on the preparation surface. Methodology: Using five different preparation techniques 75 simulated root canals in resin blocks (fifteen blocks in each group) were prepared. In group one, two and three the canals were prepared by K-flexofiles with a master apical file of size 35 using the balanced force preparation technique. After this initial preparation the canals in the second and third group were instrumented in a step back manner with 1-mm increments short of working length (second group) and 0.5-mm increments short of working length (third group). At the end of the step back preparation the root canal was flared using the master apical file in a 'push-pull' filing technique. In group four and five Pro Taper was used in a crown-down preparation technique to size F2. In the fifth group a brushing motion was used with Pro Taper F2. Cross sections of the canals were made at the level of three and five millimeter from the apical foramen. Cross-sectional images were made pre- and postoperative to determine the preparation surface of the root canal. The data were analysed with AN OVA, Kruskaii-Wallis test, Mann-Whitney U test and Pearson correlation with a critical significance level of a=0.05. Results: At five millimeter from the apical foramen there was a significant difference between group one and two, one and three and four and five (p<0.05). At three millimeter from the apical foramen there was a significant difference between group one and three, group two and three and group four and five (p<0.05). There is no correlation between the measurements on five millimeter from the apical foramen and the measurements on three millimeter from the apical foramen and vice versa (p=0.320). Conclusion: Under the conditions of this study Pro Taper F2 and K-flexofiles used in a brushing motion increased significantly the preparation surface of the root canal wall at the end of the root canal preparation, at five millimeter from the apical foramen, compared to root canal preparation without a brushing motion. At three millimeter from the apical foramen however a brushing motion is of no extra value.
