Abstract

Context: Maternal thyroid dysfunction from early pregnancy onward can negatively affect neurodevelopment of the unborn child. It remains difficult to draw firm conclusions from observational studies investigating a suboptimal maternal thyroid state and cognitive functioning of the offspring, because of the use of non-objective cognitive tests, pre-school age at assessment and small sample sizes. Objective: We aimed to investigate the possible relationship between maternal thyroid functioning during pregnancy and school performance of the 5-year-old offspring. Method: School performance was measured using test scores of the national standardized school achievement test (Cito-test) Arithmetic and Language in first and second grade of primary school. Maternal thyroid function was evaluated using thyroxine (free T4) and thyrotropin (TSH) levels at the end of first trimester of pregnancy. Results: In both the Arithmetic and the Language test a significant higher incidence of poor test scores was found for the children of hypothyroxinemic mothers. Maternal hypothyroxinemia (i.e. a free T4 concentration in the lowest 10% of distribution) was found to be associated with a below average school performance on the Arithmetic school test (Odd’s Ratio 1.889, 95%-CI 1.264 – 2.823, P= 0.002). After adjustment for family background and perinatal conditions, this association remained significant. No such general association was found at the Language school test. However, without adjustment for family background and perinatal conditions, a significant association of a free T4 <10th percentile and a TSH >90th percentile was seen at the Language test too. Conclusion: Maternal hypothyroxinemia at the end of the first trimester of pregnancy was associated with poor school performance, specifically a below-average school performance Arithmetic, of the 5-year-old offspring.
