Abstract
Objective: Several factors are associated with the development of the metabolic syndrome and weight gain in severe mentally ill patients. An unhealthy lifestyle is one of the few risk factors that seems amendable to change. The objective of this study was to examine the effects of an exercise-based lifestyle intervention on physical activity, the different components of the metabolic syndrome and weight change. Methods: The study had a group randomised design and it had a pragmatic character. All patients from 29 long-term care teams of GGZ Friesland and Lentis Groningen were included in this study and were randomised per team into either a control or intervention condition. Teams in the intervention condition received a lifestyle intervention that was delivered by lifestyle coaches whose task it was to transfer skills and knowledge about healthy lifestyle choices onto the patients and their carers/nurses. After these three months there was a nine month follow-up period in which the nurses had to take over the task of the lifestyle coaches. The control groups received care as usual. Measurements were taken at the beginning (T0), immediately after the three months of intervention (T1) and after nine months of follow-up (T2). Results: The lifestyle intervention was successful in significantly improving the objectively measured physical activity at T1. Weight and factors associated with the metabolic syndrome were not significantly different between the intervention- and control group nor was it different between the group that did and the group that did not improve their physical activity. Conclusion: The study showed that a lifestyle intervention leads to a significant increase in physical activity compared to the control groups on the short term. The intervention did not show a decrease in cardiometabolic health factors. The study showed no significant long term effects. A different study design with a prolonged intervention from the lifestyle coaches may be needed to enable long term lifestyle-effects.
