Abstract

Background: Groin complaints are a common problem affecting the patient’s physical functioning and quality of life. Adequate measuring instruments for the assessment of physical functioning and quality of life in patients with groin complaints are lacking. The Hip and Groin Outcome Score (HAGOS), a Patient-Reported Outcome (PRO) questionnaire for people with groin pain was therefore developed in Denmark. The original HAGOS consists of six subscales: Pain, Symptoms, Physical function in daily living, Physical function in Sport and Recreation, Participation in Physical Activities and Hip and/or groin-related Quality of Life. The purpose of this study was to translate and cross-culturally adapt the Hip and Groin Outcome Score into Dutch (HAGOS-NL) and to evaluate the internal consistency, validity and reproducibility of the translated questionnaire. Methods: The translation and cross-cultural adaption of the Dutch version of the HAGOS were performed according to international guidelines. Clinimetric properties of the HAGOS-NL were evaluated in terms of internal consistency, validity and reproducibility. The study population consists of 151 adult patients who had received surgical treatment at the Medical Centre of Leeuwarden for a groin hernia between 01-10-2011 and 01-09-2012. All respondents filled in the HAGOS-NL, the Short Form-36 (SF-36) and the Short Musculoskeletal Function Assessment (SMFA-NL). To evaluate reproducibility, 51 respondents who were treated at least 6 months ago, filled in the HAGOS-NL for the second time after 1-3 weeks. The clinimetric properties of the HAGOS-NL assessed were: internal consistency (exploratory factor analysis and Cronbachs alpha), construct validity (Spearman’s rho), and reproducibility (intraclass correlation coefficient and Bland & Altman analysis). Results Exploratory factor analysis of the HAGOS-NL demonstrated a factor solution consisting of four subscales: Pain, Physical function in daily living, Physical function in Sport and Hip and/of groin related Quality of Life. The four newly identified subscales of the HAGOS-NL and the original subscales of the HAGOS showed satisfactory internal consistency. No floor effects were seen, however ceiling effects were present in both the newly identified subscales of the HAGOS-NL and the conventional subscales of the HAGOS. High correlations were observed between similar subscales of the HAGOS-NL, SF-36 and the SMFA-NL, indicating good construct validity. The HAGOS-NL showed high reproducibility except for the conventional HAGOS subscale Participation in physical activities. There was no systematic bias between the first and second measurement. Conclusion: We successfully translated and cross-cultural adapted the Hip and Groin Outcome Score into Dutch (HAGOS-NL). This study shows that the HAGOS-NL is a valid and reliable instrument for the assessment of recovery/functional status and quality of life of patients who have groin complaints. It is recommended to use the HAGOS-NL in the evaluation of interventions for patients with groin complaints, were the patient’s perspective, physical functioning and health related quality of life are of primary interest.
