Abstract

Objectives Colectomy rate in acute severe ulcerative colitis(ASUC) patients remains high(27-29%), even in the era of biological treatment. The goal of this study is to identify predictive factors for colectomy. These predictors can aid clinicians in their decision making for acute colectomy. So, patients with a high risk for colectomy can be identified earlier, leading to a decrease in preoperative hospitalization, and therefore leading to a decrease in postoperative complications as well. Methods This retrospective cohort study was performed by collecting data of ASUC patients over a 10 year period. Data were acquired from medical charts from patients treated for ASUC in the University Medical Centre Groningen. All data were tested for crude statistical differences. Expected predictors were then analysed with a univariate and eventually a multivariate regression analysis. Results Of all 97 patients, 53 underwent colectomy. The univariate analysis identified male gender and a positive P-ANCA to be predictors of the risk of colectomy. The multivariate analysis identified extra intestinal manifestations in the joints and serum albumin as independent predictors for colectomy. Conclusions Extra intestinal manifestations in the joints and serum albumin are independent predictors for colectomy. Since scarcely any studies have investigated these correlations, further investigation is essential.
