Abstract

Objective: Prematurity is a common phenomenon, which can result in varied short and long term outcomes. Nowadays literature direct most of their attention to very preterm born children (VP; < 32 weeks). Research into late preterm born children (LP; 32 till 36 weeks) is a lot less achieved. Especially little is known about the consequences for parenting. Therefore this research compares parenting problems for VP, LP and children born at term (38 till 42 weeks). Besides determine the possible mediation of psychosocial problems in this association and the influence of the confounders gender and parental education. Study design: Parents of 402 VP, 648 LP and 386 fullterm born children, completed the Child Behavior Checklist (CBCL) at the age of four, and the parenting questionnaire at the age of five. The 12,5% highest scores on total risk score of the parenting questionnaire were considered as families with parenting problems (the remain 87,5% were considered as no parenting problems). This result is compared for VP and LP towards fullterm born children using the logistic regression. Next the total CBCL-score and the confounders are added to the regression. Results: Both VP as LP score significantly higher on total CBCL-score and total score of the parenting questionnaire. This indicates more psychosocial problems and more parenting problems for both groups. Between VP and LP, no significant differences are found. The risk of parenting problems increase for preterm born children with psychosocial problems (OR = 3,432). Gender (OR = 0,996), mothers education (OR = 1,931) and fathers education (OR = 1,763) does not appear to influence. Conclusion: Parents of VP and LP facing more parenting problems, compared to parents of fullterm born children. Psychosocial problems are more common in this two groups also. Noticeable, both outcomes are more worse for LP than VP. It looks like psychosocial problems mediated the relation between prematurity and parenting problems. The found associations barely changes when parental education and gender were taken into account.
