Iron deficiency and overload in the ELITE cohort

Abstract

Background: Dyshomeostasis of the iron metabolism is a frequent phenomenon. About a third of the world’s population is affected by anemia of which iron deficiency (ID) with lowered ferritin is the most common cause. Iron overload is much rarer. It can be due to hereditary factors. Recently, its association to metabolic syndrome or dementia is frequently discussed. Methods: Data of about 3,500 unselected individuals over 16 years of age from a locoregional area in Germany have been investigated for the prevalence of lowered and elevated ferritin and correlated to lipoprotein and dementia test results in defined subgroups. Results: Concerning ID we observed a significantly higher incidence in younger females than anticipated. In other subgroups results were as expected. We did not observe an association between iron overload and pathological DemTect results. We confirmed that elevated ferritin is associated with lipoprotein disorders. Elevations of lipoprotein a levels are more likely accompanied by lowered ferritin in females. Conclusion: The prevalence of ID in the subgroup of young women is much higher than expected and may be caused by locoregional factors. The correlation of elevated ferritin levels and high levels of blood lipids should be analyzed with a view to causes and effects. As elevated lipoprotein a levels seem to be accompanied by low levels of ferritin, this should be further investigated. There was no association between iron overload and dementia.
