Abstract

The annual attrition rate of medical residents who are in training for a medical specialty is about 10%. This study focuses on the reasons for attrition by medical residents for quitting their specialization. Factors related to the resident, the training, the supervisors, the working conditions and the educational climate are taken into account. Knowledge of the above could lead to an improvement of the training climate, and such adjustment of the training that failure is reduced or prevented. This regional pilot study also aims to examine whether a large-scale national study of these factors is feasible. The study design consists of two parts: a literature review and a survey that was send to residents and dropouts. We used the data from the literature as a framework describing the current situation and we compared this framework with the results of the survey. We received data from 27 respondents, of which 10 dropouts, and 17 residents. Besides the known reasons of attrition from the literature (working conditions, lifestyle and reasons concerning the content of the job), we also found training program-related reasons. Our results suggest that the performance of the trainer(s) and the quality of the training with the training climate may be important factors that could also be a reason for the resident to quit their specialty. Insight of the reasons related to this can provide a good starting point for the analysis of attrition. We think that our findings underline the importance of the presence of a solid organization, solid working conditions, a safe training climate, a solid educational atmosphere and adequate supervision, to help in limiting reasons for attrition. There are plenty of tools available to measure the quality of these conditions and use the results to make the appropriate adjustments. In addition, our findings suggest that more attention should be given to guidance and support of the (insufficient functioning) resident and to the apparently low preparedness of the trainers to criticize their own actions and to be open to criticism.
