Abstract

Introduction Adrenal masses are frequently discovered coincidentally during imaging testing, these adrenal masses may be a pheochromocytoma. Pheochromocytoma is a rare tumor of the chromaffin cells of the adrenal medulla. It is essential to exclude these pheochromocytoma, due to heavy complications that they may cause, when an adrenal mass is coincidentally discovered. Pheochromocytoma is biochemical diagnosed by demonstrating an increase of the metanephrine level in blood or in urine. This study investigates whether pheochromocytoma can be excluded based on the density of the tumor on an unenhanced CT. This has the advantage that the patient will be less burdened. Because the blood draw (after 20-30 minutes of rest), that is required for biochemical diagnostics, is no longer necessary. In addition, the costs for a metanephrine determination are saved, accounting for about € 100,-. The purpose of this study is to determine the lowest attenuation value of pathologically proven pheochromocytomas on an unenhanced CT, so that the determination of metanephrines is unnecessary. Materials en methods A retrospective multicenter cohort study was performed, wherein the attenuation values of 69 pathologically proven pheochromocytomas were determined on an unenhanced CT. Patients diagnosed with pheochromocytomas between 01-01-2000 and 31-12-2016 were included in this study. Results The attenuation values of all the pheochromocytomas described in this study were found equal or higher than 16 Hounsfield Units (HU) on an unenhanced CT scan. Conclusion The results of this study suggest that biochemical research into a pheochromocytoma is unnecessary if the attenuation value on an unenhanced CT is lower than 16 HU.
