Abstract

Background: When considering an optimum surgical resection in breast cancer surgery an accurate assessment of the size of the tumour is important. Previous studies have shown that the size of the breast tissue excised is an indicator of the post-operative aesthetic result after breast cancer surgery. This study set out to assess to what degree measurement of the size of a tumour by a pre-operative ultrasound is useful in assessing the lump volume to be removed. Methods: The study group consisted of 72 women, who had undergone breast-conserving surgery through wire-guided biopsy or a lumpectomy over a period of six months. The size of all excised lumps was determined using a water displacement technique. At the same time the sonographic tumour diameter, the histological tumour diameter, the histological margin status and the surgeon who operated, were recorded. Results: No significant correlation was found between the sonographic tumour diameter and the specimen volume. However, a high correlation (r = 0.810) was found between the sonographic tumour diameter and the histological tumour diameter in the wire guided biopsy group. In the lumpectomy group a low correlation was found (r = 0.236). The maximal tumour diameter measured was within 5 mm of the pathologic tumor size in 99.4% of cases measured by breast ultrasound in the wire-guided biopsy group, but only in 59.4% of cases in the lumpectomy group. In the wire-guided biopsy group it was also found that the tumour masses with positive margins were statistically significantly bigger than the tumour masses with negative margins (p = 0.04). Conclusion: An accurate pre-operative assessment of the expected excised breast volume will be helpful in planning the correct surgery. Ultrasound examination of the breast is a safe, non-invasive method which seems to be especially accurate in measuring tumour size wire-guided biopsies. Given the weak correlation between the sonographic tumour diameter and the actual lump volume excised, with the accuracy of the existing pre-operative (sonographic) imaging, an accurate prediction of volume excised is not possible at this point in time.
