Abstract

Aim The results of an recent study tell us that the primary healthcare use of colorectal carcinoma (CRC) patients and mamma carcinoma patients is raised in the year after diagnosis in comparison with patients without cancer. CRC patients also show a higher healthcare use in the year before the diagnosis CRC is made. The purpose of this study is to obtain insight in the course, size and reasons for primary healthcare use of colorectal carcinoma patients in the period before diagnosis. How often do they visit the General practitioner and from which point on is a raise notable? What are the reasons for healthcare use? Methods Retrospective longitudinal study of patients selected from the database of Registration Network Groningen. Patients are matched with three controls at sex, age and general practitioner. The outcome variables are: the total number of contacts and prescriptions by a quarter of a year. Besides that the reasons for these contacts and prescriptions are studied by using ICPC and ATC codes. A research period of three years is studied. Results There are 191 study patients included who are matched with 552 controls. Patients with CRC have 94% (1.56-2.40, 95% CI) more contacts in the first quarter before diagnosis, 32% (1.05-1.66, 95% CI) in the second and 40% (1.11-1.77, 95% CI) more contacts with their General practice in the third quarter before diagnoses than patients without CRC. Patients with CRC use 22-48% more medication and this is significantly more in all the quarters of a year before diagnosis during the whole study period. The reasons for contact within primary healthcare are different for CRC patient within the following chapters: blood and blood-forming organs, digestive tract, circulatory tract and musculoskeletal tract. Study patients also get more drugs prescribed within the following ATC-chapters: alimentary tract and metabolism, blood and blood-forming organs, cardiovascular system, antimicrobial agents for systemic use, systemic hormones and musculoskeletal tract. Conclusion Colorectal carcinoma patients show a raise in healthcare use of the general practice from three quarters before diagnosis. They also use more medication during three years before diagnosis. Further research to investigate the course and reasons for medication use is necessary to enlighten a potential association between medication use and CRC.
