Abstract
This study is about High Frequency Oscillatory Ventilation (HFOV). This is a way of ventilation in which ventilation is performed with very high mean airway pressures and very small tidal volumes, even smaller then the anatomic dead space. This method of ventilation is mainly used for interstitial lung disease. These diseases require very often very high pressures from the ventilator to reach adequate oxygenation. Because of these high pressures, ventilator induced lung injury (VILI) arises. HFOV is a ventilation method that can reduce and even prevent this. This is a pilot study to find out if there are clinical parameters that identify the optimal lungvolume during the recruitmentprocess with HFOV. This to prevent overdistention. This study is performed as follows: a patient is connected to the HFOV-machine to be recruited. During this recruitmentprocess the patient is also connected to a computer which monitors and saves data from the RIP. Other variables are also saved, as SpO2, TcCO2, PaO2, tidal volume(Vt), amplitude(∆P), FiO2, en continuous distending pressure(CDP). After this, two groups are created, responders and non responders. This is done by the RIP-measurements. There were no significant differences between the two groups. It is important that this study is continued to see if there are significant differences in a bigger population to show right ways to monitor HFOV.
