Abstract

Objective: to compare the possibilities and the suitability of the DiagnoCAM™ by detecting proximal lesions in deciduous and permanent teeth in comparison with the use of radiography and visual inspection. Especially the use of the DiagnoCAMrM is examined in deciduous teeth of young children to see if the DiagnoCAMrM can be applied in de the general practice with the aim of detecting proximal lesions. Another aim of the research was to evaluate and compare the images of the DiagnoCAM™ with radiographic images and the visual inspection. Design: 42 children were asked to participate in the study. They were divided in two groups. Group A: children in the age of 5-7 years and group B, children in the age of 10-12 years. The deciduous teeth included in the research are de 54/SS, 64/65, 74/75 and 84/85, group A. The permanent teeth included in the research are the 16, 26, 36 and 46. In the first part of the study the application possibilities are evaluated. The elements are scored with the outcome of the visual inspection, the radiography images and the images of the DiagnoCAMrM. These images are compared. In the second part of the study we evaluate suitability of the DiagnoCAMTM. The children and the researcher have answer three questions. The questions are: what is the impression of the DiagnoCAMTM, the feeling during the use of the DiagnoCAMTM and the duration of the examination. The results are analyzed with IBM SPSS Statistics 19. The kappa-test and the Wilcoxon signed rank test are used. Results: 264 elements are evaluated. 192 elements in group A, 72 elements in group B. Images of the visual inspection, the DiagnoCAMrM, and the radiographic images are compared. In the first part of the study the wilcoxon signed rank test is used. The results show: DiagnoCAMTM 9 times> visual inspection., visual inspection. 42 times> than DiagnoCAMrM, DiagnoCAMTM= visual inspection.: 141 times. Group B, DiagnoCAMTMl time> visual inspection. visual inspection. 20 times> DiagnoCAMTM ®. DiagnoCAMTM= visual inspection. 51 times. Comparison iagnoCAMrM_ radiography: DiagnoCAMTM2 times> radiography, radiography 6 times> DiagnoCAMrM, DiagnoCAMTM= radiography: 8 times. Results in the second part are analyzed with the kappa test. The impression of the DiagnoCAMrM. Researcher: 33 times positive, child: 27 times. Researcher: 9 times neutral, child: 15 times neutral. no negative result .Kappa 0,545. 2). The feeling during the use of the DiagnoCAMrM. Researcher: 34 times positive, child: 28 times. Researcher 4 times neutral, child 4 times neutral. No negative. Kappa 0,447. 3) The duration of the examination. Researcher: 21 times short, child 17 times. Researcher 16 times neutral, child: 20 times. researcher: 5 times short, child 5 times short. Kappa 0,525. Discussion: Research part 1: in 73% of the cases the DiagnoCAMrM scored the same value as the value of the visual inspection. 23% of the cases gave a lower score of the DiagnoCAMrM. Because the visual inspection was set as the golden standard, the DiagnoCAMTM had a higher rate of false negative outcomes. Comparison radiography, SO% same score, 37% lower score DiagnoCAM™. Research part 2: the diagnoCAMTM had possibilities but needs specific skills and interpretation of the images. Conclusion: The DiagnoCAMTM can be used as a diagnostic instrument for detecting approximalleasions in deciduous and permanent teeth but it needs specific skills and interpretation of the three dimensional images. Because of the duration of the search and the time necessary to gain the skills needed it is not possible to give a definite answer to the question whether the DiagnoCAMTM can replace other diagnostic instruments
