Abstract

Background: Lanolin (Wool Fat) is a natural substance in a varying composition. Prolonged exposure to lanolin can induce sensitization, resulting in allergic contact dermatitis. Different lanolin products are used in patch testing. Objectives: The primary aim of this study was to determine which combination of lanolin products is most effective in patch testing for diagnosing lanolin contact allergy. The secondary aim was to identify potential risk factors for lanolin contact allergy. Methods: A retrospective analysis was performed on the records of 594 patients patch tested between 2016 and 2017. Patients were tested with Lanolin Alcohol (30% in petrolatum), Amerchol L101 (50% in petrolatum) and 11 other lanolin products. Lanolin Alcohol and Amerchol L101 were double tested. Results: Overall, 170 patients (28.6%) had a positive patch test reaction to at least one lanolin product. High reaction rates were seen for Lanolin Alcohol (14.7%, 95% confidence interval (CI): 11.3%-18.2% and 8.8%, 95% CI: 6.5%-11.0%) and for Amerchol L101 (15.0%, 95% CI: 11.5%-18.5% and 10.3%, 95% CI: 7.8%-12.7%). The addition of Amerchol L101 to Lanolin Alcohol significantly increased the number of positive cases (odds ratios (OR) 1.79, p < 0.001). An age below 40 years (p = 0.007) and atopic dermatitis (p = 0.008) were positively associated with lanolin contact allergy. Conclusions: The combination of Lanolin Alcohol and Amerchol L101 is effective in patch testing for diagnosing lanolin contact allergy. Testing with other lanolin products may not be worthwhile.
