Abstract

Introduction: Smoking is often mentioned as a contraindication for breast reconstruction because of possible negative effects on complication rates, Quality of Life (QoL) and Patient Satisfaction (PS). This study was designed to explore how the incidence of complications and QoL and PS differs among smokers compared to non-smokers. An important aspect of this work was determining the right reconstruction method for high-risk patients undergoing alloplastic breast reconstruction. Methods: For this study, data were used of 56 patients treated with alloplastic breast reconstruction at the University Medical Center Groningen. Smoking and other patient determinants of complications were analysed with use of the Pearson Chi-Square Test and logistic regression analysis. QoL and PS was investigated with a preoperative and six weeks postoperative questionnaire (BREAST-Q) for the whole study population and additionally for smokers vs. non-smokers. Results: Smokers had seven times more major complications than non-smokers (OR=6.5) and patients with BMI >25 had eleven times more major complications than patients with BMI <25 (OR=10.6) in multivariate regression analysis. Conclusion: Smoking and obesity increase the complication rate to such an extent that on these grounds patients may be denied alloplastic breast reconstruction. Smokers should cease smoking four weeks before breast reconstruction. Patients with BMI >25 who can delay reconstruction, should lose weight before surgery. This study revealed no relevant issues with QoL and PS.
