Abstract

Context: Emotions play an important role in the professional development of trainees, especially in a specific context; end-of-life care. We sought to understand how elderly care medicine trainees narrate and respond to the emotions evoked by complex situations in relation to death. Methods: Using constructivist grounded theory, we applied rich pictures and interviews both to understand the experiences and emotions of medical trainees. We included twenty-four medical trainees in elderly care medicine from two Dutch academic medical centers. The trainees drew rich pictures about complex end-of-life situations and elaborated on their experiences during semi-structured interviews. We analysed the rich pictures and interviews. Results: Participants drawings depicted a range of emotions. Most of the trainees depicted negative emotions, which arose when they were not able to provide ‘a good death’ for patients. Not being able to provide ‘a good death’ was mostly caused by factors from the context, like patient-related factors, social factors and systemic factors. Conclusions: This study reveals that there are a number of factors that can make an end-oflife situation complex and which lead to emotions of medical trainees. This research shows that elderly care medicine trainees are adequately trained and prepared for medical content. By mapping these factors, trainees can be better prepared for complex situations around the end-of-life and associated dilemmas.
